Appointments with Dr. Eidelman or Gretchen Thorner may be made by calling our office Monday, 10am – 5pm, Tuesday - Thursday, 9am - 5pm and Fridays 9am – 4pm. We suggest you schedule your appointments as far in advance as possible to ensure availability. For initial appointments, please download, complete and bring with you the attached forms (Registration Form and Medical History Form). You may also familiarize yourself with the HIPPA Notice of Private Practices. We ask that you arrive 15 minutes prior to your scheduled appointment in order to complete your registration process. Please bring a photo ID, insurance card and preferred pharmacy information with you.

We accept cash, Visa and MasterCard. Co-payments are collected upon arrival and payment in full is due at the time of service. If we participate in your insurance plan the estimated patient portion is due at the time services are rendered.

Cancellation Policy: We understand that sometimes it is necessary to re-schedule an appointment. We ask that you give at least 24 hours notice prior to canceling and we will gladly reschedule. Monday appointments should be canceled by noon the previous Friday. Please be sure to speak to our receptionist so we know how to handle your deposit. Leaving a phone message will not be considered a cancellation. If you arrive after your appointment was scheduled to begin you will be seen but only for the amount of time remaining. If you are 20 minutes late for a 20-minute procedure, this will be considered a missed appointment. Pre-paid laser packages are not refundable. In the event that you are unable to give sufficient notice, a cancellation fee will be billed to your account. We regret any inconvenience this may cause.
